RN Care Manager - Discharge Planning - Providence St. Peter campus in Olympia, WA

SCHEDULE: 1.0 FTE; 40 hours per working week. Monday-Friday, 8a-4:30p PST (flexible start and end time; no later than 8:30a start). 4 weekend shifts are to be worked quarterly.

This RN Care Manager role will predominately support Discharge Planning for multiple departments within Providence St. Peter.

Under supervision of Care Management Manager, works as a catalyst to promote 1) Coordination of care, 2) Continuity of Care, 3) achievement of consistent and/or expected clinical conditions, 4) patient advocacy and 5) efficient resource management. Coordinates the care for complex patients from pre-admission to immediate post-discharge. Acts in a non-caregiver capacity by facilitating coordination and communication between all members of the health care team, in the decision making process to minimize fragmentation of the health care delivery system. Assists health system assuring financial viability by performing comprehensive medical reviews utilizing appropriateness criteria and interpret implications of clinical status, medical treatment plan with current federal regulations guidelines; thereby expediting the billing process, payment and preventing governmental sanction. Effectively communicates (speaking, reading, and writing English) and resolves issues and conflicts. Utilizes strong organizational skills and understands the concept of integration and coordination of the multidisciplinary health team.

Required Qualifications:

  • Utilization Review, Home Health, Quality Management and/or Discharge Planning or an Associate Degree in Nursing.

  • Demonstrates a positive, professional and personal image.

  • Upon hire: Washington Registered Nurse License.

  • 3 years Nursing.

Preferred Qualifications:

  • Bachelor's Degree. Nursing or a related field. Or

  • Associate's Degree. Nursing.
