Providence Peer Navigator - Community Care Center in Olympia, Washington

Description:

Providence is calling a Peer Navigator (1.0 FTE, Day Shift) to the Community Care Center at Providence St. Peter Hospital in Olympia, Washington.

The Spec-Peer Specialists will be an essential part of day -to - day operations of the Olympia Community Care Center (PCCC) and PCCC Behavioral Health team. Provides face to face and community collaborative support to adults experiencing homelessness in Thurston County. Connects people served with available community social service options while promoting the use of natural supports and existing community relationships.

In this position you will:

  • Involved in conducting the daily tasks associated with operations out of PCCC. Schedules allotted time necessary for outreach within the community. Ability to transport patients in employee vehicle to follow up appointments medical or housing related

  • Understands payer constraints relating to appropriate community referral resources

  • Seeks supervision and consultation daily

  • Ensures compliance guidelines are followed for all Federal, State and other regulations, which include, but are not limited to: CMS, The Joint Commission (TJC), EMTALA, COBRA, and HIPAA

  • Escalates problematic or high-risk interactions to manager for assistance

Qualifications:

Required qualifications for this position include:

  • High School Diploma or GED

  • One (1) year experience in providing peer support services

  • One (1) year experience in excellent written and verbal communication skills and ability to complete documentation in a timely manner

  • Current THW certification in the state of Washington as a peer in ONE of the following capacities required:

  • Peer support specialist

  • Peer wellness specialist

  • Family support specialist

  • Family wellness specialist

  • Youth support or wellness specialist

  • Maintenance of certification through continuing education hours as outlined by the state of Washington required

Preferred qualifications for this position include:

  • One (1) year experience working with patients with chronic, acute and contingent crisis
