Providence Health & Services LPN – Transitional Care Unit in Olympia, Washington


Providence is calling a Licensed Practical Nurse (LPN) (Transitional Care, Day Shift, 0.8 FTE) to Providence Mother Joseph Care Center in Olympia, Washington. We are a community of caregivers delivering every day on our Mission to provide compassionate care that is accessible for all – especially those who are poor and vulnerable.

*This position is offering a $5,000 sign-on bonus to eligible candidates.*

In this position you will:

  • Administer total, comprehensive care to an assigned caseload based on the nursing process and the interdisciplinary care plan.

  • Conduct initial and ongoing nursing assessments of all participants/residents/patients/clients. monitors health problems and health status as needed, and implement nursing care plans.

  • Maintain shift accountabilities and ensures continuity of care through proper communication and documentation.

  • Initiate a patient education plan according to the individualized needs of the patient, in conjunction with the Primary Care Physician.


Required qualifications for this position include:

  • Graduate of an approved Licensed Practical Nurse (LPN) program

  • Current Licensed Practical Nurse (LPN) licensure in Washington State

  • Current Cardiopulmonary Resuscitation (CPR) Certification

Preferred qualifications for this position include:

  • Current Basic Life Support (BLS) certificate for Healthcare Providers

  • Prior patient care experience in sub-acute, long term care and/or with Alzheimer’s or Dementia patients

  • At least 1 year of nursing experience

Providence Mother Joseph Care Center in Olympia, Washington serves people of all ages and faiths. Our Transitional Care Unit is designed for patients coming out of the hospital who need skilled nursing care, end of life care, or rehabilitation for those recovering from surgery or who have a functional loss from illness, disease, or an accident. Our Long Term Care unit provides skilled nursing in a residential setting, addressing the needs of our residents through creating a sense of community and home in “neighborhoods” and providing a model of care that is resident-centered.
