Providence Health & Services Intensive Case Manager/Group Therapist in Olympia, Washington
Providence is looking for an Intensive Case Manager/Group Therapist (Per Diem 0.0. FTE, Day Shift) to Providence St. Peter Hospital in Olympia, WA.
We are seeking an Intensive Case Manager to provide services to outpatient psychiatric clients. They will work with the Community Care Center and home health team to identify and support vulnerable community members. Case management includes providing short term intensive care coordination with partner agencies, housing resources and health care providers. It also includes continued mental health stabilization, assisting clients' success for a given period of time in their community. They will continue to work with some clients for an extended period of time after they are transferred to adult case management services. Group therapist coordinates and conducts group psychotherapy. Elements of this position include clinical assessment of patients, brief psychotherapy, crisis intervention, referral, care coordination, and transition.
In this position you will:
Provide clinical assessments as necessary for a variety of clients with serious mental illnesses.
Modify the Intensive Case Management program activities according to need.
Plan, schedule, coordinate and lead psychotherapeutic and psychoeducational group activities.
Comply with Washington State Law and other regulatory groups regarding patient care.
Report and document group activities, individual sessions and client progress in the medical record in a timely manner.
Articulate professional issues in an organized fashion (SBAR), and contribute to problem solving and quality improvement by assisting patients in writing individual crisis plans.
Required qualifications for this position include:
Master's Degree in Mental Health counseling or equivalent.
2 years of acute care or community based mental health work.
Preferred qualifications for this position include:
Licensed Mental Health Counselor or licensed social worker.
Experience working with and/or a knowledge base of homeless services and/or substance use.
