Providence Health & Services Intensive Case Manager/ Group Therapist - Home Health Services Psychiatry Program in Olympia, Washington

Providence is calling an Intensive Case Manager/ Group Therapist to Providence St. Peter Hospital in Olympia, WA.

We are seeking an Intensive Case Manager to provide services to outpatient psychiatric clients. The Case Manager will work with the Community Care Center and home health team to identify and support vulnerable community members. Case management includes providing short term intensive care coordination with partner agencies, housing resources and health care providers. It also includes continued mental health stabilization, assisting clients’ success for a given period of time in their community.

In this position you will:

  • Provide clinical assessments as necessary for a variety of clients with serious mental illnesses.

  • Modify the Intensive Case Management program activities according to need.

  • Plan, schedule, coordinate and lead psychotherapeutic and psychoeducational group activities.

  • Comply with Washington State Law and other regulatory groups regarding patient care.

  • Report and document group activities, individual sessions and client progress in the medical record in a timely manner.

  • Articulate professional issues in an organized fashion (SBAR), and contribute to problem solving and quality improvement by assisting patients in writing individual crisis plans.

Qualifications:

Required qualifications for this position include:

  • Masters prepared mental health counselor, counselor registration or equivalent.

  • A minimum of 2 years’ experience in acute care or community based mental health work.

  • The ability to coordinate, plan and lead, ability to relate to clients and their families as well as peers, supervisors, medical staff and other professional service providers and navigate systems.

Preferred qualifications for this position include:

  • Licensed Mental Health Counselor or licensed social worker.

  • Experience working with and/or a knowledge base of homeless services and/or substance use.
