Providence Health & Services Social Worker MSW - Utilization Review in Centralia, Washington
Providence is calling a Social Worker MSW (1.0 FTE, Day Shift) to work in the Utilization Review Department at Providence Centralia Hospital in Centralia, WA.
The Medical Social Worker provides professional psychosocial assessments, counseling and discharge planning for Acute Care and the Emergency Department. The Social Worker serves as a staff and community resource and participates as a member of the healthcare team. The Medical Social Worker provides crisis intervention counseling, patient and family resource information and psychosocial support.
In this position you will:
Identify patient and/or families requiring social work and/or discharge planning intervention through established case finding mechanisms. Standards
Maintain contact with nursing, medical and other professional staff to facilitate referrals
Use appropriate rounds and team meetings to identify patient and families in need of services
Prepare social worker assessment with patient and/or family's involvement. Evaluate social, emotional and discharge planning needs
Develop specific plan to address problem and/or concerns identified in assessment, patient/family concerns, age-specific problems and clinical impressions
Provide direct one-on-one social work intervention to include but not limited to provide counseling, discharge planning, grief-loss issues, crisis intervention and behavioral issue
Required qualifications for this position include:
Master's Degree in Social Work / Arts.
Bachelor's Degree in Social work or related behavioral science.
2 years experience in Social service experience (with BA), OR1 year experience Social service experience (with MSW/MA).
