Providence is calling a Physical Therapist (1.0 FTE, 40 hours per week Day Shift) to work in Physical Therapy at Chehalis Satelite Clinic, 5 miles from Providence Centralia Hospital in Chehalis, WA.

The physical therapist in an outpatient clinic will work with a variety of patients. Patients are mostly orthopedic in nature, but can include any patient in need of outpatient physical therapy services. The Physical Therapist needs to be able to work as a team that includes physical therapy staff, local physicians, and Providence Centralia Hospital staff. In addition, he/she should work well autonomously and be willing and able to take on project development/improvement tasks.
In this position you will:
Render or supervise physical therapy treatment prescribed by a licensed provider:
Performs initial evaluations, re-evaluations, and physical therapy treatments.
Ensures that all physical therapy services are provided in a safe manner and incorporate the latest knowledge of physical therapy.
Effective in dealing with patients (or customers) of all ages. This position has regular and recurrent direct contact with Adolescents, Adults and Geriatrics.
Effective in maintaining a level of documentation that is consistent with the guidelines of the profession, of regulatory agencies, and of third-party payors.
Performs accurate and appropriate documentation on a daily basis.
Completes daily documentation in a timely manner
Required qualifications for this position include:
Bachelor�s Degree and a Graduate of an accredited school of Physical Therapy
Current Physical Therapy (PT) licensure in Washington State
AHA Basic Life Support (BLS)
