Providence Emergency Department Registered Nurse (RN) Care Navigator in Centralia, Washington

Description:

Providence is calling an Emergency Department Registered Nurse (RN) Care Navigator (0.5 FTE, Days) to Providence Health & Services in Centralia, WA.

The RN Care Navigator is responsible for managing patient populations with acute, chronic illness and/or multiple co-morbidities, identified as requiring additional transition of care support following visit to the Emergency Department setting. This individual works in coordination with multiple PCPs and/or community providers and serves as the central link to an identified population of patients. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the ED RN Care Navigator assists patients to gain self efficacy/management skills, achieve optimum functional health status, and quality of life. The ED RN Care Navigator assists patients/families, staff and systems to achieve high quality, evidence based, cost-effective, and patient focused outcomes. This person works under minimal supervision within the general policies and standard practices and the nursing code of ethics, including Chapter 18.88 RCW.

In this position you will:

  • Provide leadership and continuity of care guidance for patients with acute, chronic diseases and/or complex disease states. Uses knowledge of illness, diagnoses and treatments in comprehensive, holistic assessments of patients. Performs patient teaching, encourages patient self-management/self-efficacy coaching and planned care management, and ensures transitions of care coordination for specialty and other handoffs as necessary:

  • Utilizes appropriate resources to gather data and assess patient care needs; makes ongoing assessments of the following domains: physio-biological, cognitive/emotional, spiritual and social, managing life with chronic disease; identifies problems that could lead to an emergency/crisis situation and takes appropriate action to de-escalate; identifies the need for and coordinates specialists/consultants; communicates assessment data to appropriate persons; assesses learning needs of patient/significant others; documents assessment data according to clinic standards.

  • Initiates discharge phone calls to identified patient population to ensure smooth transition of care and adoption of identified treatment plan.

  • Plans interventions appropriate to patients’ medical and nursing diagnoses, age, abilities and resources; establishes and implements learning and disease self-management plan for patient/significant others; documents planning process according to organizational standards.

  • Provides teaching to patients/families related to patient’s diagnosis, pathology, medical and nursing treatment plans, discharge needs and health goals; documents each element of care per organizational and unit standards.

  • Collaboration, delegation, supervision:

  • Collaborates and communicates care with patient’s PCP and/or community provider.

  • Forms partnerships with all members of the interdisciplinary team.

  • Communicates with coworkers and other departments in a professional manner; shares appropriate information with all members of the health care team, including assistive/support personnel, in a manner that is appropriate and timely for the patient’s condition.

  • Participates in team huddles.

  • Monitors patient and family satisfaction.

  • Identifies, documents and reports patient safety and/or quality incidents per PMG standards.

  • Supports and guides emergency department care teams with implementation of evidence–based practice guidelines.

  • Assists in designing and implementing planned care interventions through a combination of evidence-based guideline/pathway utilization and individualized care planning.

Qualifications:

Required qualifications for this position include:

  • Graduation from an accredited school of nursing.

  • 2 years RN role in the Emergency Department setting

  • Currently licensed as a Registered Nurse (RN) in the State of Washington

  • Current BLS certification – Healthcare Provider (must be AHA accredited)

Preferred qualifications for this position include:

  • Bachelor's Degree in Nursing

  • Experience managing complex patients in an Emergency Department setting

  • Certified Emergency Nurse (CEN)
